APPARENT SYMPATHETIC RESPONSE OF CONTRALATERAL NON-LENS WEARING EYES AFTER OVERNIGHT LENS WEAR IN THE FELLOW EYE

Desmond Fonn, Renee du Toit, Ping Situ, Jose Vega, Trefford Simpson, Robin Chalmers* Centre for Contact Lens Research, School of Optometry, University of Waterloo, Canada *CIBA Vision

Purpose:

To compare central corneal swelling (CS) and light scatter (LS) after 8 hours of sleep in eyes wearing high and lower Dk lenses and control eyes.

Method:
20 neophyte subjects wore lotrafilicon A (Dk 140, HiDk) and etafilicon A (Dk 28, LoDk) hydrogel lenses of similar center thickness overnight in random order in their right eyes only. Naked left eyes served as controls. CS and LS were measured before lens insertion, after lens removal upon waking and every 20 minutes for the next 3 hours using an optical pachometer and the psychophysical method of the Van den Berg Staylightmeter.
Results:
CS and LS induced by the LoDk lens was significantly higher than with the HiDk lens (CS: 8.6%±0.6% vs 2.7%±0.4%, p<0.001 and LS: 13.8%±3.0% vs 9.5%±2.4%, p=0.025 respectively). The swelling of the naked control eye paired with the LoDk lens was also slightly but significantly higher than that of the control eye paired with the HiDk lens (2.3%±0.3% vs 1.4%±0.2%, p=0.0015). LS measurements were not significantly different between control sets of eyes (6.5%±2.0% vs 5.6%±1.9%, p=0.05), but showed the same trend.
Conclusions:

In an overnight swelling experiment with neophyte subjects, the corneal swelling and light scatter in the naked contralateral control eyes appears to be influenced by the swelling and scatter in the fellow experimental eyes. Because the same apparent effect was demonstrated by two different methodologies, this may reflect either an unusual sampling coincidence or a real though unexplained sympathetic physiological response.
